การตรวจสุขภาพประจำปี 2025

รายละเอียดและหลักเกณฑ์

พนักงานที่มีสิทธิ์ได้รับการตรวจสุขภาพ และฉีดวัคซีนป้องกันไข้หวัดใหญ่ มีดังนี้

• พนักงานประจำและพนักงานชั่วคราวที่ ผ่านการทดลองงานภายในวันที่ 30 เมษายน 2568
• ช่วงเวลาการตรวจสุขภาพจะเริ่มในวันที่ 7-31 พฤษภาคม 2568

ขั้นตอนการตรวจสุขภาพ

ก่อนถึงวันตรวจสุขภาพ

• ลงทะเบียนเลือกวันตรวจสุขภาพตามตารางตรวจสุขภาพที่กำหนด ผ่าน QR Code ที่แจ้งไว้
• สามารถเลือกซื้อรายการตรวจสุขภาพเพิ่มเติมได้ที่  QR Code ตามที่แจ้งไว้
• ได้รับ SMS แจ้งเตือนเมื่อใกล้ถึงวันตรวจสุขภาพ

วันตรวจสุขภาพ

• งดน้ำ-งดอาหารอย่างน้อย 8 ชั่วโมง

หลังตรวจสุขภาพ 1 เดือน

• รับผลตรวจสุขภาพ Online ผ่าน QR Code ที่ลงทะเบียน
• ได้รับเล่มตรวจสุขภาพ

กรณีที่ไม่สามารถรับการตรวจตามวันและเวลาที่บริษัทกำหนด

• สามารถเข้าตรวจได้ในช่วงวันที่ 1–30 มิถุนายน 2568 เวลา 08.00 - 15.00 น.
• โดยเลือกสถานที่ตรวจฯ เป็น “โรงพยาบาลซีจีเอช” มี 3 สาขาให้เลือก(พหลโยธิน สายไหม ลำลูกกา) ผ่าน QR Code เดียวกัน

รายการตรวจเพิ่มเติม 
พนักงานที่ต้องการตรวจสุขภาพเพิ่มเติม สามารถดูรายการและชำระค่าใช้จ่ายด้วยตนเองผ่าน QR Code

เพิ่มเติมรายการตรวจสุขภาพให้เหมาะกับช่วงอายุ

การตรวจสุขภาพจะถูกจัดขึ้นในช่วงเดือนพฤษภาคมของทุกปี โดยในปีนี้ได้เพิ่มเติมรายการตรวจสุขภาพที่เหมาะสมกับช่วงอายุของแต่ละบุคคล เพื่อให้สามารถติดตามสุขภาพได้อย่างมีประสิทธิภาพ จากเดิมรายการตรวจสุขภาพเหมือนกันทุกคน เปลี่ยนเป็นตามช่วงอายุ 

ขั้นตอนการลงทะเบียน

1. กรอกรหัส พนักงาน พนักงานประจำให้กรอก THxxxxตามด้วยรหัสพนักงานสี่หลัก ส่วนพนักงานชั่วคราวต้องกรอกรหัสที่บริษัทของท่านกำหนด
2. กรอกชื่อ-สกุล เป็น ภาษาไทย หรือ ภาษาอังกฤษ
3. กรอกหมายเลขโทรศัพท์เพื่อรับ SMSแจ้งเตือนเมื่อใกล้ถึงวันตรวจสุขภาพ
4. กรอก Email เพื่อ รับผลตรวจสุขภาพ online
5. ตอบคำถามให้ครบทุกข้อ
6. ติ๊กยินยอมการเปิดเผยข้อมูล
7. คลิ๊กลงทะเบียน

 การดูผลตรวจสุขภาพ

1. กรอกชื่อ-สกุล เป็น ภาษาไทย หรือ ภาษาอังกฤษ
2. คลิ๊กดาวน์โหลด

 ANNUAL HEALTH CHECKUP 2025

 Conditions & Eligibilities

• Permanent & Third party staff who passed probation within 30 April 2025
• The health examination period will be from 7-31 May 2025.

Health Check-up process 

Before the Health Check-up:

• Register and select a health check-up date via the QR Code.
• You can purchase additional health check-up items using the QR Code below.
• You will receive an SMS notification as the health check-up date approaches.

On the Health Check-up Day:

• Fast for at least 8 hours prior to the check-up.

After the Health Check-up (1 Month Later):

• Receive health check-up results online via the registered QR Code.
• You will also receive a health check-up booklet.

If You Cannot Attend on the Scheduled Date:

• You can attend any time between June 1-30, 2025, from 08:00 AM to 03:00 PM.
• Choose "CGH Hospital" as the check-up location, with 3 branches available (Phahonyothin, Sai Mai, Lam Luk Ka) via the same QR Code

From this year onwards, the annual health check-up program will change to include health check-ups according to age groups, as announced in the health check-up program.

Registration process

1. Employee codes for permanent employees must be in accordance with THxxxx. The four-digit employee code for temporary employees must include the code specified by the company.
2. Enter your first and last name in Thai or English.
3. Follow the news to receive SMS notifications. when the health examination date is approaching
4. Enter your email to receive health check results online.
5. Answer all questions completely.
6. Tick to connect data.
7. Click to register.

Health checkup report

1.  Enter your first and last name in Thai or English.
2. Click Download.
